Please read the following information carefully, as it will explain
the procedures for your first few visits to our centre.

Day #1 – Today

Complete our Patient Information form (or bring your completed form with you)

Official tour of our centre
We will perform an examination to find out if you have Subluxation
Examination includes chiropractic tests, a detailed case history and x-rays (if necessary)
X-ray referral will be to Radlink Jurong West
The cost of x-rays is $169, if a full spinal set is needed
You will be given a Family Posture Evaluation Card to complete at home.

Follow Up Appointments

These vital appointments are booked on consecutive days, scheduled either Monday/Tuesday or
Wednesday/Thursday. It is requested your partner, spouse or guardian attend bother appointments.
It is, however, suggested that he/she attend the second of these visits.

Day #2 – Report 1- The Problem

Please return the completed Family Posture Evaluation Card
Eight minute, orientation video – improves your understanding when viewing your x-rays
The Doctor will reveal the results of your examination and x-rays
If your case has been accepted, you will receive your first adjustment. The cost of an adjustment is
$75, payable on the day.

Day #3 – Report 2 – The Solution

Your Partner/spouse/guardian attendance is very important
Doctor will reveal recommendations and protocol for your care
Future appointments and cost-effective payment schedules will be mapped out
You will be asked to make a decision, including a financial decision, regarding your treatment plan
Should you choose to go ahead with the recommended treatment plan, you will receive your second
adjustment. This adjustment will be part of your treatment plan and does not require individual
payment.

Payment Policy

The retail value of the new patient examinations and reporting process is $240
This payment is payable on Day #1.
If you have attended our office with a gift certificate or in response to a promotion, the advertised
fee is due and payable on Day #1. (The advertised amount does not include the adjustment at Day 2)

Patient Information Form, and return it to one of our Chiropractic Assistants. Our collective goal is
